• Patients diagnosed with dementia by their doctor

• Patients with Medicare Part B. Patients are not eligible if their primary insurance is Medicare Advantage or another Medicare health plan, including Special Needs Plans and the Program of All­ Inclusive Care for the Elderly.

• Patients must not be a long-term nursing home residents (defined as a nursing home stay of four months or more)

GUIDE is provided at no-cost to patients and caregivers and there are no co-pays for the program.

Respite, for eligible families, is also provided at no cost for up to 80 hours per year.

Craniometrix is a CMS-approved, certified GUIDE participant and is able to provide access to all of the program’s benefits.

We follow an evidence-based model (which GUIDE was derived from) called MIND at Home.
