
Introducing GUIDE
The Guiding an Improved Dementia Experience (GUIDE) model is a new Medicare program designed to support the unpaid caregivers of patients with memory or cognitive symptoms. The program is fully funded by Medicare for enrolled patients with no co-pays or out-of-pocket costs.​​

The GUIDE model gives you and your loved ones access to respite care, a dedicated medical team, and caregiver support. At the center of the model is your family's Johns Hopkins-trained Care Navigator, who will guide you through the care journey.

Fully covered by Medicare
If your family member has been diagnosed with dementia and is enrolled in Medicare Part B, you can receive all the above benefits at no cost to you. Medicare has also waived the normal 20% co-pay for all GUIDE services, including respite care.
GUIDE Benefits
Medicare-funded respite care
Everyone deserves to take a break. Respite care offers caregivers a chance to rest while a licensed day care center or trained home care aide provides care. As part of the GUIDE model, you may be eligible for up to 80 hours of respite care each year.
Personalized Care Plans
Our specialist care navigators will perform a detailed assessment upon enrollment in GUIDE and at least once a year. Your medical team will create a personalized treatment plan for you or your loved one.
A personal care navigator
Your personal care navigator will be a partner in navigating dementia care and will check in with you or your loved one regularly. They will provide personalized support and referrals to community-based services like meals and transportation
24/7 Support
As a member of our GUIDE Model program, you will have access to dementia-trained specialists and support staff through our 24/7 support line.
